Ready to Join Our Family?
Follow these three simple steps to secure your child’s spot:
1. Check Availability
Before starting your paperwork, please contact your preferred campus directly to confirm they have an open spot for your child’s age group.
2. Secure Your Spot
Once availability is confirmed, you’ll need to pay the non-refundable registration fee. Enrollment is limited, and priority is given to families who have completed this payment.
3. Complete Enrollment
After paying your fee, fill out the online enrollment package and submit it to the Campus Director.
Enrollment Forms
These are the necessary enrollment forms that must be completed and returned as soon as possible.
Necessary Forms for Admission:
The attached forms MUST be completed and returned to Smarty Pants Pre-School.
- Childcare Application for Enrollment CF-FSP Form 5219
- Contract between Smarty Pants Pre-School LLC. and Parent/Guardian
- Emergency Information/Notification
- Authorized Child Pickup
- Alert Form
- Field Trip General Permission Slip
- Permission to Photograph Child
- DH Form 3040 Student Health Examination (Page 2 completed by Pediatrician)
- DH Form 680 Certification of Immunization Part A1 – B or C
- DH Form 681 Religious Exemption from Immunization
The forms highlighted in red must be completed and provided by your child’s pediatrician or the Florida Department of Health.
Children cannot be admitted without the certification of the Immunization Forms.
